Innovation in Community Outreach Award

The Innovation in Community Outreach Award recognizes physical therapists, physical therapist assistants, and students who have demonstrated exceptional creativity and effectiveness in developing and implementing initiatives that advance population health, prevention, health promotion, and wellness within their communities.

Purpose

This award, presented by the Population Health Special Interest Group of the Academy of Leadership and Innovation, celebrates those who embody the group's mission to connect people, knowledge, and resources for the betterment of society's health.

Eligibility
Any current member of the APTA Academy of Leadership and Innovation Population Health Special Interest Group.


Award Criteria
Recipients of the Innovation in Community Outreach Award will be selected based on their demonstrated excellence in the following areas:

  • Innovative Approach: The initiative should showcase a novel or highly creative approach to addressing a community health need. This could involve new partnerships, unique delivery methods, or inventive uses of existing resources.
  • Impact and Outcomes: Clear evidence of the initiative's positive impact on the target population and community. This includes measurable outcomes related to health promotion, disease prevention, improved access to care, or enhanced well-being.
  • Replicability and Sustainability: The initiative should demonstrate potential for replication in other communities or settings, and ideally have a plan for long-term sustainability beyond the initial implementation phase.
  • Collaboration and Partnerships: Evidence of successful collaboration with community organizations, other healthcare professionals, or stakeholders to achieve shared goals.
  • Alignment with Mission: The initiative must clearly align with the mission of the Population Health Special Interest Group, demonstrating a commitment to advancing population health, prevention, health promotion, and wellness for all members of society.

Deadline:

  •  Nominations must be submitted no later than November 14. 

Presentation:

  •  Awarded annually at the Academy reception at CSM.
