As healthcare moves from volume to value in the skilled nursing and home health setting, there will be challenges to organizations and individual providers. The ability to collaborate with teams across disciplines to optimize health outcomes is essential.  Applying team science knowledge to work processes will help improve interprofessional team effectiveness to achieve excellent patient centered outcomes.  

In this webinar, the presenters will address the challenges inherent in working with people from a variety of professional backgrounds. Using the knowledge of team science, process improvement  theories and evidence of successful partnership development in healthcare and other businesses, they will provide information and tips to help teams move the bar to sustainable interprofessional practice.

About The Speakers:

Lori Pearlmutter is the Director of Population Health for North Central Arizona Accountable Care, an ACO comprised of two hospital systems, multiple primary care and specialty practices. As a physical therapist with three decades of experience in healthcare leadership, she has worked with an array of interprofessional teams throughout the continuum to achieve high quality care and patient engagement.  In addition to presenting at numerous state and national conferences for PT and other professions, she has served on the board of the Acute Care Academy and in positions with the Arizona Physical Therapy Association.  Presently, she is the treasurer of HPA-the Catalyst. Lori has an MPH, is certified in health care quality, and is a patient centered medical home content expert. 

Dr. Tina Gunaldo is the director for the Center for Interprofessional Education and Collaborative Practice at Louisiana State University Health Sciences Center-New Orleans. She has engaged in IPE for the past seven years. In addition to presentations and publications, Dr. Gunaldo serves on the Louisiana Immunization Workgroup supporting a collaborative approach to increasing immunization rates, as well as contributes to the development of the Louisiana Area Health Education Center's (AHEC) Scholar curricular program.  Most importantly, Dr. Gunaldo is supported by a wonderful team of over 100 faculty trained in IPE, who have supported and/or integrated IPE into their academic practice.
